This person will be responsible for both facilitating individuals with disabilities to become more integrated with their community and to find and maintain competitive employment. Education: Bachelor’s degree preferred or Associate’s degree in a related field acceptable. Ideal candidate would possess qualities such as dependability, high energy, creativity, and be self-motivated. Individual must be able to work independently and also as a team member.
Valid driver’s license with a good driving record required per agency policy and current auto insurance required to transport persons served
Create a Community Integration Plan for each individual assigned
Prepare and write case notes for each client
Participate in required annual trainings
Create a portfolio of community activities to align with outcomes in client ISP
Promote the Community Participation program to community and other entities
Foster relationships with area businesses to build base for client employment
Assist client in performing work responsibilities and ability to foster independence
Perform task analysis of each task as needed and provide guidance to client
Conduct community based work assessments
Perform other duties of the department as required and assigned to accomplish goals
Comply with agency rules regarding confidentiality, housekeeping, safety, and legal standards
Required to obtain Basic Employment Services Certification or Employment Services Professional Certification by 1/01/2019.
This position is full-time, Monday through Friday, 8 AM to 4:30 PM. but may be occasional evening hours and Saturdays, based on client’s needs.
